Aggregated attach() config-push failure surface — one entry per host that didn’t acknowledge installation.

HM-4: per-host identity is preserved so callers can act per-host. The host-identity key is HostRef (the iteration unit in push_config()); the per-host cause is a ConfigPushFailure (taxonomy is implementation-detail; see the type’s doc).
